Evidently Chang has misunderstood my request. Porrino's Pearls are no joking matter. This is a very offensive gesture on his part, and serious damage has been done.

With that said, Porrino' Pearls, a daily commentary by your's truly, Jack Porrino Jr., will one day be legitimate. A place where people will be able to read a short synopsis on the day's events, putting things into perspective.

Chang's lack of respect is not unexpected, but nevertheless, very disappointing.

My very first blog is likely one of great interest to many. I would like to briefly discuss Mr.R Schneider. Over the past year, there has been overwhelming buzz over his "approach", originally published in 1975 (see reference below). So why you ask has this forgotten technique suddenly re-emerged as the "preferred" method of performing fluoroscopy guided shoulder arthroscopy on the streets in 2009? Well, honestly, I am not sure I have the answer for that one.

In summary, the approach, as described by a very reputable colleague of mine, is rather simple. Simply take a 20-22 gauge spinal needle, and jab it into the glenoid labrum. Once you have confirmed iatrogenic creation of one of Resnick's 26 types of SLAP tear (by way of listening for a characteristic "crackling" sound, or feeling crepitus-like return in your hand), vigorously inject the full 20 mL solution into the joint.

Seems a bit renegade and careless? Maybe. Yet it remains the talk of the department, no matter where I turn. I strongly encourage you to become familiar with Mr. Schneider's original description. You will not be sorry.

An easy and reliable technique to inject the shoulder joint for arthrography is described. The technique utilizes a straight anteroposterior position of the patient and a directly vertical placement of the needle with the aid of fluoroscopy.
